Pre Employment Questionnaires

Before you take on an employee, do you ever consider the medical, mental and physical state of the employee before you employed them. If not, the implications to your business can be catastrophic.

The Disability Discrimination Act prohibits an employer from discriminating against a person on the ground of their disability, even at the recruitment stage. The employer may therefore need to make reasonable adjustments to workplace, the working process or even place an individual in another role. Failure to do so can lead to civil claims and prosecution.

Healthwise are able to provide you with a simple process to ensure the employee is fit to do the job and at the same time you meet your duty of care – what’s more the process is simple !!

Postal Health Questionnaire

1. Healthwise will supply a pre formatted health questionnaire form. Section 1 is completed by the Recruiting Officer, section 2 by the potential employee.
   
   
2. The Recruiting Officer completes Section 1 of the questionnaire and gives it to the candidate for completion with a reply paid envelope.
   
   
3. The candidate privately completes the questionnaire and returns it in a pre paid reply envelope
   
   
4. On receipt an Occupational Physician reviews the responses in relation to the job function. A report on fitness, unfitness, recommendations or restrictions is then provided to the Recruiting Officer within a maximum of 2 working days.

Fees

£25.00 plus VAT per questionnaire received and reviewed.  Where there is a query on a particular questionnaire that requires a physical consultation or GP report, further charges will apply.

Or the other option is an only line facility at a cost of £15.50 per assessment

Service standard

All pre-employment questionnaires will be reviewed and reported on within 2 working days.
