£25.00 plus VAT per questionnaire received and reviewed. Where there is a query on a particular questionnaire that requires a physical consultation or GP report, further charges will apply.
Or the other option is an only line facility at a cost of £15.50 per assessment

All pre-employment questionnaires will be reviewed and reported on within 2 working days.
